Proof of Covid-19 Vaccination

This form is for attendees of the 9th annual Expression of KINDness 

As noted at registration, proof of COVID-19 vaccination is required for all attendees. To expedite your check-in process on Thursday, October 7th, we are asking all registered attendees to submit photos of their vaccine cards and complete the credit card authorization form prior to Tuesday, October 5th. This will allow us to pre-check all attendees and will eliminate delays and long lines when you arrive for the evening’s festivities — because once you arrive at Bigsby’s Folly for your dining experience, we want your evening to get started as quickly and smoothly as possible!

On this page we kindly ask each attendee:

  1. To submit their proof of Covid-19 vaccination prior to the event. 
  2. Download, fill out, and upload the Credit Card Authorization Form for on-site bidding, bottle pulls, etc.
  3. Let us know if you have any dietary restrictions for the dinner. 

Please submit your proof of vaccination and authorization form by Tuesday, October 5, 2021.

The time has passed to submit your Credit Card Authorization Form.  We do ask that you still download the form, fill it out and bring it with you.  This will greatly aid the check in process.
